  Application for exemption from Attendance

Application for exemption from Attendance at School under 5 days.

  Application for Extended Leave

Application for extended leave if longer than 5 days, each application needs to be submitted to the Principal Leader for consideration. If approved your child will be notified in writing with a letter from the Principal.
